Case Management Supervisor
Position: Case Worker
Employee Level: Full Time
Supervisor: Case Management Supervisor
Pay Range: $22-$24 (based on education and/or experience)
Hours: Full Time: 8:30am-5pm Monday – Friday
Exodus Place – Michigan’s largest nonprofit transitional housing program for men coming from
homelessness, incarceration, and other life challenges - is seeking an energic individual with high
work ethics and experience in the Case Worker Field to provide resource-based services to its
members. This position will work collaboratively with the Exodus Place team to coordinate and
monitor the assessment, treatment planning process and follow up of clients while maintaining
the integrity and culture of our programs. Duties will be inclusive of but not limited to the following:

ESSENTIAL DUTIES AND RESPONSIBILITIES
Essential Functions/Activities:
• Maintain current and accurate case files.
• Conduct Life Skills Classes (i.e., budgeting, resume building etc.)
• Conduct member activities (i.e., movie nights, game nights etc.)
• Aid clients in the formation of linkages between relevant agencies and organizations.
• Assist clients to obtain benefits (e.g., Bridge Cards, External Case Management, My
Health, housing, and employment).
• Provide informal crisis intervention for assigned caseload.
• Address clients’ needs and progress promptly and follow-up to ensure that appropriate
services are delivered.
• Set goals with clients and maintain a consistent follow up on those goals.
• Submit required weekly and monthly reports to supervisor in a timely manner.
• Attend inter-agency staff meetings, training/presentations, case management meetings,
and case consultations as required.
• Represent Exodus Place in a way that shows the program's integrity, the culture of our
organization.
• Must maintain confidentiality of employee, client, and other information.
• Must be able to demonstrate compassion and grace while maintaining healthy boundaries.
• Ability to rotate with other staff members for after-hours calls.
Requirements
• A Passion for working with the homeless population and recently incarcerated
encouraged.
• Ability to work as a team.
• Detail oriented
• Must have a cell phone available for work.
• Elevated level of communication skills verbal and written
• High work ethic and attention to punctuality
• Proficient in Google and Microsoft Office
Education and Experience
• Community Health Worker Certified a plus
• Bachelor's in human sciences
• 3-5 years' experience in the human services field
